Diabetic foot is a complication of diabetes. These patients require a special approach to foot care, since with impaired sensitivity and insufficient blood supply in lower extremities any damage to the skin means a longer recovery period. Unfortunately, improper foot care or failure to follow doctor’s recommendations, can lead to further inflammation, ulceration, and even gangrene. It is especially important for these patients to choose right shoes. In the vast majority of cases tight shoe is a contributing factor to the development of ulcers. Right shoe selection with the help of a podiatrist will speed up the healing process. Patients also sometimes find it difficult to take care of their feet on their own, because diabetes is often accompanied by excess weight, decrease in vision, neurological disorders, etc. Ingrown nail, foot fungal diseases, dry skin, calluses, hyperkeratosis, darkening nail plate – all these are cases when diabetic foot should be consulted with a doctor. This is why it is necessary to visit a podiatric care center for diabetic foot.
Podiatric care (instrumental pedicure) for patients with diabetes is the safest and most atraumatic option for the treatment of feet and nail plate. It allows to remove hyperkeratosis and calluses without damaging the surrounding healthy tissue. Nails and skin are treated with special care, paying attention to all the problematic areas.
The foot treatment consists of several stages:
1. inspection and sterilization of feet
2. events calendar elaboration
3. hyperkeratosis removal using special heads
4. feet final polishing
5. care, by the means of special creams
Instrumental pedicure is performed only by an experienced podiatrist with medical knowledge of the diabetic patients’ foot treatment. To prevent complications patients should follow podiatrist instructions.
Podological care of the feet for patients with diabetic foot
For the majority of patients with diabetes nutritional disorder of the feet in the region of bone and joint is noticed. These changes are manifested by increased keratinization of the feet skin – places of maximum pressure. This not only helps in forming hyperkeratosis, but also subsequently contributes to formation of cracks- which later on will become focies of easy pass for potential infections. The changes are caused by alterations in blood supply, as well as pathological damage of nerve fibers (diabetic neuropathy). Decreased sensitivity causes disturbances in the functioning of the sebaceous and sweat gland of the feet, which leads to severe dryness, and subsequently to the formation of hyperkeratosis.
Even in cases, where changes in the condition of the foot are not noticed, observation of patients suffering from diabetes by an orthopedist is still necessary. The specialist examines and evaluates the main types of sensitivity, evaluates the blood supply, which allows identifying patients at risk at an early stage, and starting treatment ahead of the appearance of ulcers. The specialist also treats the foot: removing manifested hyperkeratosis, treating ulcers; indicates the need to reduce the load on the affected limb; in the absence of contraindications, prepares individual unloading bandages.
Patients suffering from diabetes mellitus, who are not in the risk group, are recommended to undergo an examination at least once a year. In the presence of complications, confirmation of impaired blood supply to the lower extremities, medical examinations should be carried out more often: at least 2-4 times a year. Even more often, if there are complaints or indications.
